Ten Copies

Clerk's desk/August 30, 2026

The name or cause is wrong

Corrections are a vital-records errand, not a photocopy job. Forms differ by state. Banks want a new certified copy after the record is fixed.

If the spelling, date, or cause of death is wrong, do not white-out the certified copy. The office that keeps the original record has to amend it. There is no single national form.

Who usually files what

  • Cause of death and medical items usually go back to the funeral director, the certifying clinician, or the medical examiner, as that state requires.
  • Name, date, and similar identity items usually go through the state or local vital records office on that state's affidavit or application.
  • Use CDC's Where to Write for Vital Records and the state desk for the jurisdiction of death. Do not mail a Florida affidavit to Illinois.
  • After the amendment posts, order new certified copies. Insurers and banks want the amended certified copy, not the first paper with the error.

This desk does not process amendments and does not give legal advice. Confirm the current form and fee on the official vital records site for the state of death.
