Maricopa County deaths
A Phoenix-area death is a Maricopa County vital-records copy. The state Bureau of Vital Records is the other Arizona desk, not the first window for a recent local death.
If the death was in Phoenix, Mesa, Scottsdale, or the rest of Maricopa County, start with Maricopa County Public Health, Office of Vital Registration. The county issues certified copies of Arizona deaths, not only deaths inside Maricopa County. The Arizona Department of Health Services Bureau of Vital Records in Phoenix is the statewide backup.
What the county page listed
- $20 per certified copy. There is no cheaper extra on the same request. Corrections are listed at $30. Confirmed 3 Sep 2026 on maricopa.gov.
- Arizona is a closed-records state. You must be at least 18 and document your relationship. Read the county eligibility page before you pay.
- Valid government-issued photo ID plus the relationship papers that page lists.
- Mail: PO Box 2111, Phoenix, AZ 85001. In person: 301 West Jefferson Street, Phoenix, and other valley offices on the county locations page. All offices closed Monday 7 Sep 2026.
Pima County (Tucson) is a different health department. Do not mail a Maricopa death to the state first unless the county page sends you there. Online orders, where offered, go through a vendor. This desk takes no cut.
