Maricopa County Public Health issues certified copies of Arizona deaths, not only deaths inside Maricopa County. The state Bureau of Vital Records in Phoenix is the other desk. For a recent local death, start with the county counter.
Who may request
Arizona is a closed-records state. The applicant must be at least 18 and must document their relationship to the person named on the record. See Maricopa County's death-certificate eligibility page for the current list.
What to bring
Valid government-issued photo identification, plus the relationship documents the eligibility page lists. Confirm the current list on maricopa.gov before you go or mail.
Where to write or visit
Mail: PO Box 2111, Phoenix, AZ 85001. In person at 301 West Jefferson Street, Phoenix, AZ 85003, and other valley offices listed on the county locations page.
Notes from the desk
The county lists $20 per certified copy. There is no cheaper extra on the same request. Corrections are listed at $30. Order extras on the first request so you do not wait twice. Online orders, where offered, go through a vendor. This desk takes no cut.
