Miami-Dade county health issues Florida deaths from 2009 to the present. Deaths before 2009 go to the Bureau of Vital Statistics in Jacksonville. Jacksonville extras are cheaper on the state page this check ($4 each additional). Confirm both fee schedules before you choose a counter.

Who may request

A family member or legal representative, especially if you need the cause of death. Florida treats cause of death on recent records as confidential. Confirm eligibility on the county application and on the state Bureau page if you mail to Jacksonville.

What to bring

A legible copy of a valid driver license, passport, state-issued ID, or military identification must accompany the request, per the county health department.

Where to write or visit

In person at the Downtown, North Dade, or West Perrine locations listed on the county health page. Walk-in hours on that page are Monday through Friday, 8 a.m. to 4 p.m., no appointment. Use the county death-certificate application for mail.
