Hennepin can issue deaths that happened in Hennepin County (1870 to present) and deaths in any Minnesota county from 1997 to present. The Minnesota Department of Health is the statewide backup. Start with Hennepin for a Minneapolis death.

Who may request

Minnesota death records are public, so anyone may buy a noncertified informational copy. A certified copy is a different product. Confirm who may request a certified copy on health.state.mn.us (Who Can Order Records) and on the Hennepin application. Banks want certified.

What to bring

In person: driver's license, state ID, or passport. If you have no ID, a witness can complete the county statement of identity. Mail requests for certified copies must be notarized. Confirm on the current application.

Where to write or visit

Mail the county application with check or money order to the address printed on that form. In person at Hennepin County service centers (appointment or walk-in as listed on hennepincounty.gov). Fax is offered with an extra fee.

Notes from the desk

County page this check: certified copy $13, each extra $6. Noncertified is also $13. Expedite by mail is an extra $20 if you write Expedite on the envelope. Fax adds $9.50. Online orders go through VitalChek. That processing fee is not the $13 copy fee. This desk takes no cut. Minnesota Department of Health lists the same $13 certified-copy fee at all vital records offices.
