Fairfax County is not the independent City of Fairfax, Falls Church, or Alexandria. Place of death still controls the record. Any Virginia local vital-records office, including Fairfax County Health, can issue a Virginia death copy. Richmond is the statewide mail backup.

Who may request

Yourself or immediate family: mother, father, spouse, adult child, adult sibling, grandparents, adult grandchildren, with valid ID. Legal representatives need papers. Confirm on fairfaxcounty.gov and vdh.virginia.gov.

What to bring

One ID from the Virginia primary list, or two from the secondary list, at vdh.virginia.gov/vital-records/id-requirements/.

Where to write or visit

Annandale, Franconia-Springfield, Herndon-Reston, and Mount Vernon public health centers listed on fairfaxcounty.gov/health/vital-records. Payable to Fairfax County Health Department. If the local search fails, the request goes to Virginia Division of Vital Records in Richmond. DMV full-service offices also issue Virginia vital records.

Notes from the desk

$15 per certificate or search this check, even if no record is found locally. Virginia local offices can issue statewide deaths (June 1912 to present). This desk takes no cut.
