Tacoma-Pierce County Health for a Pierce death. Washington DOH is the statewide backup when the local office does not have the record.
Who may request
Long-form: spouse or domestic partner, parent, child, sibling, grandparent, grandchild, legal or authorized representative, next of kin if no one else on the list is living, or the funeral director named on the record (time-limited). Short-form adds title insurers and people who can show a property or personal-right need. Confirm on tpchd.org.
What to bring
One current government-issued ID (not expired more than 60 days), or two documents from the alternative list on the health department page.
Where to write or visit
Order in person, by mail, or online as listed on tpchd.org. Some records are sent to Washington State Department of Health. Confirm the counter on that page before you drive.
Notes from the desk
Certificate fee is $25 per record this check. Walk-in, mail, and online each add processing (and VitalChek online). Those extras are not the certificate fee. This desk takes no cut. King County is a different health department.
