The Register of Deeds issues Wisconsin death certificates from 1 Sep 2013, including deaths outside Milwaukee County. Older certified copies are a different search. Genealogy death records at this office go back further. Do not mail the city clerk.
Who may request
Wisconsin restricts certified copies to people with a direct and tangible interest. Confirm on the Register of Deeds vital-records page and on the application before you pay.
What to bring
Mail: a copy of a valid picture ID (driver's license or state ID) with the application. Confirm the current list on county.milwaukee.gov.
Where to write or visit
901 N. 9th Street, Room G6, Milwaukee, WI 53233. Mail the application with a copy of valid picture ID. In person Monday through Friday, 8 a.m. to 4 p.m. at that counter.
Notes from the desk
County fee this check: $20 first copy, $3 each additional of the same record. In-person orders add a $3 service fee. Those extras are cheap. Order them on the first request. The City of Milwaukee is not the death-copy counter. This desk takes no cut.
